2. Complete & Submit our Lending Library Request Form*

3. We will respond to your request form within one business day to confirm pick-up/drop off times**

* Please give us ≥ 48 hrs notice for requests to ensure equipment and personnel are available

** Pick up/drop off times occur on Tuesdays & Thursdays

Things to Remember:

Read, print, & sign the liability waiver and borrowing agreement
Bring your signed forms & student card when picking up your items
Clean your items before returning them
